We have omitted some minor choices that have very minimal to no impact on the consequences. So without any further delay, let’s begin our choices and consequences guide list.

Baldur’s Gate 3 Choices & Consequences Guide

Mind Flayer Pod Choices

There is a choice to either interact or not with the pod in Mind Flayer.

If you interact with the pod

If you interact with the pod, it asks you to save a trapped woman survivor. She is a companion in Baldur’s Gate 3, whom you will meet shortly after interacting with the pod. Hence, if you interact with the pod and rescue her, she will remember it and become a great companion.

If you do not interact with the pod

If you opt not to interact with the pod and not rescue the woman, it will impact your relationship with that companion.

Remove or Destroy the Brain

The second choice you get in Baldur’s Gate 3 is in the second room aboard the vessel at the Prologue area. When you hear the sound and interact with the Neural Apparatus, you will be transported to another level in the vessel where a body with its brain revealed will await you. Now you have the choice to either remove or destroy the brain.

If you remove the brain

The consequence of removing the brain in Baldur’s Gate 3 offers two other choices. If you choose this option, you will be asked to either spare or mutilate the brain. If you spare the brain, the creature falls to the ground. On the other hand, if you choose to mutilate, you will interfere with the creature but it won’t notice and become a companion.

If you destroy the brian

When you opt for this choice, you destroy the brain completely, and the human host dies.

Survivors in the Mind Flayer

You can try to persuade survivors in Mind Flayer’s mind. If you succeed, they will fly, and if you fail, they will start attacking you.

There is another choice to make here. You can talk to Mind Flayer here and are given two choices upon that. The first choice is to break free and the other choice is to concentrate on his thoughts. If you opt to break free, you can then kill Mind Flayer from outside. If you opt for the latter choice and succeed to concentrate, there are two more choices given.

The next two choices you get are either to submit or resist by using Wisdom. If you submit, Mind Flayer will consume your brain and the adventure will come to an end. On the other hand, if you resist, you will have to fight against the Mind Flayer.

The Old Lady and Two Brothers

You get a total of three options when you meet the old lady and two brothers. They are in the middle of an argument and you can either choose to kill the two brothers, save them, or do nothing at all. If you are wondering why would you kill the two brothers, then it’s because the old lady will offer you removing the parasite in exchange for your eyes.

If you choose calm down

This option will calm down the situation and allow you to save the two brothers. But this will make the old lady upset and she will vanish. You will then be given a quest by the two brothers to save their sister.

If you choose lower your weapons

This option will ask the two brothers to lower their weapons and your character will kill them.

If you choose nothing to do with me

This option will keep you out of the argument, but you won’t be able to save the brothers. The old lady will kill them and flee.

Meeting Astarion

When you meet Astarion, he will tell you about a brain bug that he has seen. You will have the choice to kill the bug or ask him to do so. In both cases, he will attack you with a knife. However, if you choose the latter option, there is a chance to escape his knife by performing a strength roll. He will later become a companion in the game.

Kill or Let Live the Owlbear

When you encounter an Owlbear, an option to wither kill or leave it is given. If you go very close to the Owlbear, the fight will begin and then you will have no choice but to kill it. However, if you stand relatively close, a conversation will begin. If you convince the Owlbear to let you leave the cage, you can simply do so. On the other hand, if you are not able to convince to let you leave, you will have to again fight it.

If you kill the Owlbear, you will have further an option to either kill or let the cub live. It dies if you kill it, and if you let it live, it starts eating the Owlbear. You can get valuable rewards from the Owlbear’s nest if you are able to kill it.

Let Nettie Poke You

When you arrive at Nettie she will offer to heal you by poking with a stick. The stick is actually covered with poison. If she is able to poke you, you will have to fight her to kill her or steal the antidote. On the other hand, you can dodge when she tries to poke you. In that scenario, you will be able to leave the room whenever you wish.
