Yes, you should take the poison from Nettie in BG3 if you don’t want to kill her. You must swear to her that you will swallow the Poison if you feel any symptoms of transforming into a Mindflayer. As soon as you select the “I swear” option, she will hand you a vial of Wyvern Poison, and thank you. This decision will gain approval from Lae’zel and Shadowheart. As you talk to Nettie, she will also give you clues to find and rescue Halsin, the Master Druid.
If you choose to not take the poison from Nettie, she will impatiently try and make you swear the word. At this point, you will get two chances to keep your word and take the Poison vial, peacefully. But if you refuse to take the poison from Nettie in Baldur’s Gate 3, she will turn hostile and attack you. Due to her low stats, you can beat Nettie with a couple of strikes.
