Since you want to extract the Illithid parasites fiddling with your mind, choosing to evolve into a half-Illithid can be pretty confusing. With the series of surprising events, the Emperor or the Dream Visitor will offer you the Astral-touched tadpole at the end of Act 2. Yes, you should become Half Illithid in BG3 if you want to use the best Illithid powers. Soon as you consume the Astral-touched Tadpole, it will transform you into a Half-Illithid. While this partial Mind Flayer evolution unlocks all the Tier 2 Illithid powers and skill tree, it also changes your physical appearance. Due to this Mind flayer transformation, you will grow veins all over the body. But the only drawback of transforming into a Half Illithid is that all of your party members will disapprove.

You can also choose to reject the Emperor and not consume the Tadpole. If you change your mind, you can use the Astral-touched Tadpole in your Inventory at any point. But if you want to unlock more Illithid powers and not lose any approvals from your party, you can convince them into consuming the Tadpole for enhancing Illithid powers.

Fortunately, becoming a Half Illithid in Baldur’s Gate 3 doesn’t affect the storyline or ending. But letting the Emperor evolve you into an Illithid or a complete Mind Flayer has an impact on the ending.
