Elden Ring Millicent Quest Guide
To begin the Elden Ring Millicent quest, you need to speak with Gowry at his shack in Caelid. During this conversation, Gowry will ask you to find Millicent. If you accept to do so, you will need to unlock the barriers at Selia to reach the Plague Church.
Once you make your way here, you need to speak to Millicent. After you do so, you will need to acquire the gold needle from Commander O’Neil. Head into the swamps of the game to find the commander. Upon coming across him, you need to fight and defeat him. Doing so will give you the concerned gold needle.
You need to take this needle and give it to Gowry at his shack. Reload the game and speak to Gowry. He will then give you the gold needle, and you will need to give it to Millicent.
Upon handing over the gold needle, Millicent will give you the Prosthesis-Wearer Heirloom Talisman. This talisman helps increase intelligence, making it quite a helpful item.
You need to now speak with Millicent at Gowry’s Shack, after which you can simply reload Elden Ring to speak to Gowry again.
Millicent’s Quest Location 1 – Altus Plateau

After you hand over the prosthetic, head over to Windmill Village. Here, you need to first defeat the Godskin Apostle boss, after which you will find Millicent at the Point of Grace. You need to speak to her here too.
Millicent’s Quest Location 3 – Mountaintops of the Giants
Once at the Mountaintops of the Giants, you need to activate the Grand Lift. Once you do so, you can go to the Ancient Ruins and speak to Millicent.
After this interaction, Millicent will head to the Halleck Tree. To get here, you need 2 Secret Medallions. You will receive one from the Village of the Albinaurics. You will get the second medallion at the Castle of Saul. To receive this medallion, you will first need to defeat Commander Niall.

Once done, head over to the Prayer Room in the Halleck Tree area and speak to Millicent. Now, go to the Drainage Area.

If you choose to help Millicent, you will receive Rotten Winged Sword Insignia. This enhances damage on consecutive attacks. Along with this talisman, you will also receive the Unalloyed gold needle. The needle is crucial to one of Elden Ring’s endings. If you pick this choice, you will need to defeat Millicent’s sisters. There are 4 of them, and you need to enter her world to defeat them.
