If you want to find Black Mushroom in Diablo 3, you will have to head into Cathedral Level 1 on Normal Difficulty. While you will have to fight a few enemies, it will be a fairly easy task if you have luck on your side. Since one of the difficulties is that it is a rare item, you might not be able to get it in one go. You might have to reset and enter the location once again and go through the ordeal of defeating enemies.
However, once you manage to get Black Mushroom, you will be able to craft Staff of Herding and gain access to Whimsyshire. Since there are multiple possible spawns, we cannot exactly pinpoint the location but if it has spawned, you will get it on Level 1 in the Cathedral.
