The Palace of Pleasure is a side-area that can be accessed through mostly luck. You need access the Tide Pool first to get to the Madame Tusk’s Palace of Pleasure. You can get to Tide Pool while defeating Olmec in Spelunky 2. When you are facing Olmec, there is an option to forfeit the fight and get to Tide Pool by pushing the wall that is covering the door of Tide Pool.
Once you are in the Tide Pool area, you will find Madame Tusk. Now all you need to do is play her Dice Game, which you can access from either 4-1 or 4-2 biome. While playing the Dice Game, you need to roll 7 four times to get the grand prize and the tag of “high-roller.”
