Towards the end, you will be given the option to become a Mind Flayer to control the Netherbrain. However, Karlach will intervene and let you know that she would want to be the one who turns into a Mind Flayer, considering she has nothing to lose anymore. The decision here will be up to you. You can either let her become a Mind Flayer or become one yourself. However, we would recommend not letting Karlach become a Mind Flayer.

This is because once she transforms, she will no longer be the same. She will no longer feel like going to die, and she will also survive once she destroys the Netherbrain. However, Karlach can die after defeating the Netherbrain as well. The possibilities in this situation will entirely depend on the choices you make. Additionally, if you choose to let her turn into a Mind Flayer, you will also get the option to betray her, take control of the Netherbrain, and become the Absolute.

If you want to save Karlach, the best option would be to let her return to Avernus with Wyll or the player instead of letting her transform into an Illithid. If you don’t choose this, the flames of her infernal engine will take over her, and she will die.

Lastly, as mentioned above, although there is a chance that she might survive after her transformation, once Karlach becomes a Mind Flayer, it will change her. Although this change won’t be drastic, she will no longer possess the personality she had during the entire storyline.
