In Diablo 4, while doing the Sister Octavia questline, Holy Chalice is an item that you will need repeatedly which happens to be bugged and players are looking for a fix. Holy Chalice Item Always in Quest Inventory Fix for Diablo 4?
To fix the Holy Chalice bug you have to complete the quest beside the portal. Thanks to Zift on Blizzard forums to suggest this fix that worked for them. You can check out the entire thread here.
In this bug, the Holy Chalice will keep showing up at the gate of Kyovashad. This bug could happen if you completed part of the questline in one world tier and the remaining on another difficulty. And if you don’t pick it up then the item will just respawn the next time you log in. This happened to be the case for Everbliss on Blizzard forums, where it kept showing up for them in World Tier 4.
Another player javabeans, said the item “keeps flinging itself from my inventory”. And they even completed the entire questline of Sister Octavia.
So the best way to avoid encountering this bug is by either complete all 3 Sister Octavia side quests in the same World Tier. Use the above fix. Or you can wait for the developers to release a patch that fixes this issue.
In case you have already completed the quest chain but it still appears in your Quest Inventory. Then you can simply ignore the item there.
