If you have played the previous installments of Gears of War you might remember the Lancer’s chainsaw bayonet, the developers have kept the same layout for this weapon too. You need to hold down the RB button on the console.
Clicking it once will only reload the weapon so make sure that you press it down for a couple of seconds before letting go.
As you hold down RB you will see a laser sight, you need to now aim at your target and let go of the button.
This will release the grenade in the air, before finding its target and creating an explosion raining down on your enemies from above.

When you have your enemy insight and a red laser is visible it means that you cannot use the Grenade Launcher of the Lancer at that moment, it could be for any reason but if a red laser is visible you should check surroundings which could be obstructing from firing the weapon.
Simply change your position and try aiming again, if you see a blue laser it means that you are in the clear and can fire the weapon.
