Once you are on the side mission On the Blood Path you will come at a crossroads where you will have to pick a side between Ramsden and Holder. Your decision here matters as both of them have different loot depending whom you select.
If you choose Ramsden over Holder, then you will have to take out Holder and his minions too, once you do that you will be rewarded with the Fingerbiter shotgun and it comes with the following perks +108% Weapon Damage, 10% Critical Hit Damage, +25% Reload Speed and 3.5x Weapon Zoom.
If you decide to side with Holder then you will have to take out Ramsden and his minions and you will get a shield that gives you a 180% boost in melee damage when destroyed.
Both these rewards are okayish and you can certainly find better weapons and shields but if you have to pick one it is better to get the shotgun.
